The following forms are required for all new faculty appointments:

All employees are required to complete I9/E-Verify certification within three working days.

• The employing department or unit would complete:
  • Personnel Action, PD-7 if person is not in Banner
  • Telephone Verification of Credentials
• The employee would complete, sign, and date:
  • Post-Employment Personnel Data, PD-7a
  • Certification
  • Direct Deposit Information
